In the US, access to GLP-1 weight-loss treatment depended on who paid, which diagnosis appeared on the prescription, and whether coverage stayed in place.
Employer and commercial plans set their own formularies. Even when they covered treatment, prior authorization often applied, so similar patients could receive different coverage decisions.
The federal drug benefit excluded weight-management-only prescriptions, but coverage could apply when the same class was prescribed for diabetes, cardiovascular risk reduction, or sleep apnea.
Under the joint federal-state program, obesity-drug coverage varied by state and managed care plan, so moving could change eligibility, formularies, and which conditions qualified.
Cheaper compounded copies largely disappeared once shortages were resolved, so uninsured patients faced brand-name pricing and should plan with prescribers before any coverage change.
