Medicare and Medicaid shape care for many US patients, so early-career physicians benefit from understanding coverage rules, eligibility differences, and daily practice impacts.
Medicare mainly covers people 65+ and some younger patients with disabilities or renal disease, while Medicaid serves eligible low-income groups; some patients qualify for both.
Medicare Part A covers inpatient-related care, Part B covers physician and outpatient services, and Part D covers medications; patients choose Original Medicare or Advantage.
Original Medicare lets patients use any Medicare-accepting clinician, while Advantage plans use networks, often include drug coverage, and may add vision, hearing, dental benefits.
Physicians entering practice should secure an NPI, complete needed Medicare or Medicaid enrollment, learn dual-eligible billing, and expect payment and authorization rules to matter.

Medicare and Medicaid Basics for Physicians

A 60-year-old with $230K saved faced a 5-yr gap before federal senior health coverage, while a common withdrawal rule yielded only ~$9.2K yearly.
Working ~20 hours weekly at a major coffee chain unlocked employer health coverage, and one worker example showed the company paying ~80% of premiums.
Current marketplace coverage looked tougher: enhanced tax credits had ended, older adults could face higher premiums, and insurers sought a median ~18% increase for 2026.
Keeping paycheck income and benefits in place could help preserve the 401(k), reduce withdrawals, and support waiting longer to claim retirement benefits.
Key move: compare the employer plan with the federal exchange, include deductibles, and protect the 20-hour average so coverage stays intact consistently.
